Cleaning up location, loading truck with scrap metal

While loading a truck with a piece of flowline, Jim Farris was rolling the flowline off of the forks of the telehandler, to make sure he didn’t damage the truck. The forklift operator, Chris Robinson (Highlands Drilling) lowered the flowline without notice pinching Jim's right hand between the flange on the flowline and the trailer.

Drilling Operations: Walking on location looking for panic lines for the choke.

While walking around on location looking for some panic lines for the choke, the injured Patterson employee stepped in a low spot and reached out to to prevent himself from falling. He reached out to grab the top of the junk tub and cut his right index finger. See photos attached.

Making rounds on compressor. Taking readings and shooting valve and discharge temps.

Mark called me (John Landis) on Friday morning and stated that his left hip was hurting and he was limping around on it. I asked Mark what had happened and he told me that he was climbing up onto the compressor to close the Louvers. He stated that he felt a pinch in his left hip and thigh but continued working. When he woke up on Friday morning he had pain in his left hip and thigh. I told Mark to go home to rest up his leg, he worked until 3:00 PM and went home. He tried working the rest of the day, but the pain persisted on Friday. Mark also told me that earlier in the week (Tuesday) he hit his left hip on a valve at the Boggess Pad. We are unsure if this has anything to do with the injury, but wanted to make note of it. Mark is currently off resting his hip/thigh.

Attaching camlock hose fitting together to lengthen suction hose.

While putting together two hoses a burr went through Cornelius's glove and cut his left ring finger. General first aid requiring antiseptic cream and a band aid was administered. No further action was required.

Was putting together two hoses and felt something scrape his finger through his glove. Took gloves off to see what had happened and saw his finger was cut. Immediately notified supervision.

Well Tender

Jeff Krakau had called me at 7:50 stating that something may have happened to Dave Beckner (he had stopped talking on phone after hearing him say "oh"). Jeff tried calling back but no answer. Jeff called me and asked if I had talked to him. I called and texted Dave, but no answer. I decided to drive out to the dehy location to check on Dave to see if he was ok. When I arrived on location Dave was reclined in his seat in his truck and holding his left hand. His arms were folded across his belly/chest. I then tried to get Dave to respond to me and after a couple of minutes he was responsive. Dave could not talk and did not know who I was. I could see a large bump on his forehead, a scratch under his left eye. I then called 911. 911 came and took Dave to Ruby Hospital. While at Ruby they did a CT scan and saw no bleeding on the brain. As a preliminary finding they were saying they thought it to be a concussion, but more testing would take place. They were going to do an MRI and an EEG on Dave. Dave does have loss of memory and cannot speak at this time.

We have no statement from Dave at this time. Jeff Krakau was talking with him on the phone when he heard Dave say "Oh" and then nothing more from Dave.

Move Woodford's 500 gal diesel tank with JLG telehandler.

Mike Shatley picked up the fuel cell using the JLG telehandler and started to back off containment without a spotter. With his wheels turned the forks swung around and knocked over a 55 gal drum of Castrol oil. Puncturing a hole in the drum he immediately got out of the telehandler and ran over to move the drum upright so it would stop spilling. He placed his hand over the sharp edges of the puncture cutting his pointer, middle, and ring fingers through his cotton work gloves. Mike went into the crew shack to wash and bandage his lacerations. He left location to receive stitches at Med Express in Waynesburg, Pa. ~1/2 gal of oil spilled off containment and ~1 gal on containment with Highlands crew immediately cleaning it up.

I was trying to pick up the fuel cell. I wanted to back straight out but the wheels were already turned. When I backed up the forks swung around and I saw the drum get knocked over. I immediately got out of the telehandler and put my hand inside the puncture to help me place it upright. The sharp edges cut through my gloves cutting my three fingers.

Tong Cable Repair/Rig Crown

Employee Todd Lee went to the crown of the rig to check the tong cable in the shive, The driller Eric Stanley left the rig running in case he needed to pick up the tong with the winch line to take the weight off the cable to put it back in the shive. After determining the winch was not needed for this task he started up with the winch to hook on the casing being cut on the table, at this time Todd Lee put his hand on the cable and got his right middle finger pulled into the shive crushing the tip of the finger. He was taken to the hospital and received 4 stitches, took off the nail and broke the bone in the tip of the finger. A saftey stand down was preformed and a follow up will be conducted. Updates will follow as we get them.

Containment Repairs

Employee Zack Anderson was operating a extrusion welder when he lost his grip on it and instinctively reached out to catch it with his other hand, by doing so he grabbed the heating element causing burns to the tips of his left thumb, index and middle fingers.

He was wearing gloves at the time. He applied ice on location but declined to go to the hospital.
Small blisters appeared on his finger tips.

He has 1 year experience and is the supervisor of the crew.

Breaking down setting tool

At approximately 4:30 AM, on April 29th, 2017, NES was operating wireline on the Northeast Natural Energy Kassay location. NES employee was breaking down setting tool and while trying to expose piston, encountered trapped pressure, which caused the tool to not stroke. NES employee tried to make tool stroke by slamming the tool on surface containment, and while in the process, got finger pinched in the setting tool. Employee later sought medical attention at local ER.

Frac pump maintenance

"Cory Wilson, a Producers Service Corporation employee, was standing on a frac pump while replacing valves and seats. He was using a hammer to remove the fluid end discharge cap. Cory slipped and fell back on the iron rack that is mounted on the frac truck. He has some minor skin abrasion on his back and side, and said he has soreness in his ribs. Cory immediately called Axiom, and did not seek any medical attention. Axiom recommended he put ice on his ribs for swelling and to take aspirin for any pain. Axiom will perform a follow up today, and Cory will seek further evaluation if needed. Producers has labeled this a first aid. Cory will be going on days off, returning Monday for dayshift.

This occurred at 10:50 pm on 2/14/18. The weather was 48 degrees F and raining."

Changing filters on outlet filter of compressor.

While changing filters Roberts hand got stuck between cap and lock down bar. He had bruising and skin abrasion. First aid was administered. He was taken to DR and it was reported as just bruising and sore. No work was missed.

While working to assist Nick form NNE and learning to change filters on the Kassay Pad, I was injured in the process of threading the end cap back on. I was pushing from the bottom with my right hand and as I pushed off one of the ears that hold the ratcheting mechanism into place my hand slipped off shooting past the hook on the ear which caught the meat of my palm pushing my hand into get crushed. Nick and I backed off the cap where Nick proceeded to check on me and provide me with a cold pack to try to mitigate the swelling and figure out what to do after.

Warren was climbing down out of his truck bed, after strapping his load down.

In the process he lost his balance and fell to the ground striking his left elbow and injuring his left shoulder. He was taken to St Josephs ER for evaluation, where x-rays determined that he had a left shoulder contusion. He was cleared to return to work 3/24 with no restrictions.
