He was new to this rig, not new to oil & gas, but Woodford was short on drivers due of the holiday. The rig was in the process of laying down the BHA so all rig hands were busy, and supervisors were on the rig floor. The driver went ahead to fuel the rig's tank. He was not aware that there was a filling inlet near ground level. Instead, he stepped up on the ladder rungs of the tank to look for the filling spout. Woodford dispatched another fuel truck driver to the pad. The new driver finished fueling the rig. The injured driver returned to the Woodford shop in Elkins. Later in the day he felt pain in his back, so he drove himself to the Elkins emergency room to get checked. Initial tests show he has a concussion and 2 fractured ribs. The injured driver is home and doing well. Jeff tried calling back but no answer. Jeff called me and asked if I had talked to him. I called and texted Dave, but no answer. I decided to drive out to the dehy location to check on Dave to see if he was ok. When I arrived on location Dave was reclined in his seat in his truck and holding his left hand. His arms were folded across his belly/chest. I then tried to get Dave to respond to me and after a couple of minutes he was responsive. Dave could not talk and did not know who I was. I could see a large bump on his forehead, a scratch under his left eye. I then called 911. 911 came and took Dave to Ruby Hospital. While at Ruby they did a CT scan and saw no bleeding on the brain. As a preliminary finding they were saying they thought it to be a concussion, but more testing would take place. They were going to do an MRI and an EEG on Dave. With his wheels turned the forks swung around and knocked over a 55 gal drum of Castrol oil. Puncturing a hole in the drum he immediately got out of the telehandler and ran over to move the drum upright so it would stop spilling. He placed his hand over the sharp edges of the puncture cutting his pointer, middle, and ring fingers through his cotton work gloves. Mike went into the crew shack to wash and bandage his lacerations. He left location to receive stitches at Med Express in Waynesburg, Pa. ~1/2 gal of oil spilled off containment and ~1 gal on containment with Highlands crew immediately cleaning it up. | Mike Shatley | I was trying to pick up the fuel cell. After determining the winch was not needed for this task he started up with the winch to hook on the casing being cut on the table, at this time Todd Lee put his hand on the cable and got his right middle finger pulled into the shive crushing the tip of the finger. He was not wearing his impact resistant gloves. Joey Richardson(drilling Superintendent) was already on site due to the time it had taken for repairs and they began the Axiom/HSE process. As soon as HSE arrived on location, they transported him to the hospital ER for treatment. He received stitches and a split as he broke his finger.
