| Summary | While carrying 2 2" fittings to hook up the casing fill up line, Kyle was walking across the floor and tripped over the hydraulic feed lines to the power tongs. When he fell he landed on his Left shoulder and separated/dislocated it. |
| Witness Statement | While walking across the floor carrying fittings Kyle tripped over the hydraulic hose and fell onto his left shoulder, he instantly felt pain and had to be assisted back to his feet. He was unable to move his shoulder and required assistance down the stairs to the rig managers house where he explained his symptoms. |
