Near Miss Reports

At approximately 1600 PM on 4/9/17, Best Line Equipment rentals was delivering a tele-handler to the Kassay Pad. The driver, Justin, had lowered his drop trailer and unloaded the tele-handler. Justin then attempted to raise his drop trailer. He then noticed the trailer was not working and had released approximately 15-20 gallons of hydraulic fluid onto the Kassay pad. The roustabout also noticed it at the same time and put a duck pond under the trailer. The roustabout continued to contain the hydraulic fluid and the driver reported it to me. We immediately contained with peat moss and began to dig up the fluid and put it a waste dumpster. We have contacted Max with NGE to evaluate the material and the cleanup. I have attached before pictures and we will follow up with after pictures when we can.

While pressure testing for stage 20 on the Kassay 7H a wing ? on the back of one of the pumps failed catastrophically. Test pressure was 10,657 psig at the time of failure. No one was in the red zone during the incident.

I received a complaint from Dave Dillie on July 3,2017. He alleges NNE has caused the floor in his trailer to start to wave, He said he checked the frame, where he could access it with the level, but agreed the frame was level. The allegations started that by drilling under it we changed the water table, which then caused the trailer to settle. After further discussion and the talking through the process of drilling, he claimed it was the sand truck traffic beating on the ground that caused the trailer to settle. I told him I would discuss it with the office and we would get back to him. 7-6-2017 I went out to look at the fence that we needed to reinstall and his daughter asked what we had decided on the trailer. A tandem from Lightening trucking was unloading the compressor off the bed, the bridal didn't have enough slack in it so it came off the ear of the skid. When it did the radiator of the compressor was bent. No one was in the red zone and no other equipment was damaged.

At 6:45 pm an Aveda truck driver was unloading his last load of the day unassisted and without authorization. As the pump for the rig was being hoisted up the roller board the line broke dropping the pump approximately 7'. No one was in the area but the pump sustained damage. It bent the skid, which throws the pump out of alignment, damaged the electrical motor and damaged the gear end. It will be about four days before a replacement can be put together.

The employee has been terminated for not listening to instructions and going against policies of all three companies involved. After the incident a safety stand down was preformed.

Today at approximately 1115AM a panhandle Casing truck was delivering 9 5/8? casing to the Coastal pad and on his way back through the fisher location, the rear of his trailer hooked the front fender of the Rig Managers truck pushing it into the safety shack. The red Chevrolet pickup is a Patterson company vehicle driven by Patterson #334 rig manager Jeff Donahue

Travis was dumping onto the stockpile with A John Deere 250 Artic rock truck. he was slightly downhill, when he dumped the load, the load did not slide out of the truck and it tilted the cab over. there were no injuries and minimal damage to truck.

On January 14th at 10:45 PM while moving the Nine Energy grease trailer from the Coastal Pad up the steep hill towards the Fisher Pad, a dash light came on in the Nine Energy pickup truck indicating "Transmission Too Hot" and the engine shut off. The truck began coasting backwards downhill with no steering or brakes. The trailer turned and pulled the pickup into the ditch, then came free from the fifth-wheel ball, which parted the safety chains and the break-away latch. The pickup came to a stop in the ditch, while the trailer continued coasting downhill. The trailer remained upright as it rolled backwards and stopped right before the tree line approximately 70 yds from the lease road.

Dustin Merchant was the driver of the Nine Energy pickup. He is a hot shot driver for Nine. He was uninjured, but was taken to the hospital for examination. Dustin also underwent a drug and alcohol test last night and the results were ?negative?.

There were no releases involved with the incident. Damage to the pickup includes body damage, driveshaft and possible hidden damage. Damage to the PCE trailer appears to be minor. The trailer has been towed back to the lease road and inspected carefully. Both the truck and trailer have been removed from NNE property. Nine Energy's investigation is ongoing.

Fracing Stage 42 on the Lemley 1H

During 2.0 lb 100 Mesh on stage 42 on the 1H the turbine caught fire internally. A loud noise was heard from inside the data van, and smoke was seen coming from the turbine. All equipment was immediately shut down. Power Generation also called to shutdown. At this point flames were seen coming from the exhaust of the turbine. Gas supply was shut in, and the 1H and 3H wells were shut in. All nonessential personnel were sent to the muster area, and the onsite NNE consultant called 911 and NNE management. The fire was contained inside the turbine structure, and some small spot fires were visible on the containment from leaking oil. The fire suppression system inside the turbine actuated and smothered the fire internally. The small spot fires were controlled with fire extinguishers. Once the fire department arrived, the fire had already been suppressed. Once the turbine structure cooled to 100 deg F, the fire department opened the turbine doors to confirm the fire was completely out.

Hauling logs to landing area with log skidder. Skidder upset.

Russell Beach (304) 692-9552 was operating a log skidder with a load of logs. The skidder upset on its side as a result of the heavy load and un even terrain.

"Dragging logs into landing he turned down road bank. The logs were still up on road and he lifted back of skidder up. Skidder fell onto its side"

Truck Driver

Tractor and Trailer hauling telehandler off site did not adhere to “no left turns” sign and turned left onto Rt 7. A rock truck came around the turn on Rt. 7 from the left having to brake excessively to keep from causing an accident. No injuries or damages reported.

Around 9:20am, a lowboy with large equipment was turning left out of the Boggess well pad entrance. As the lowboy was turning out, a stone truck was traveling north on Rt 7 and when they came around the corner they had to slam on their brakes and came close to impacting the lowboy. Mike was 2 trucks behind the stone truck and had to slam on his brakes as well to avoid an accident. Mike relayed that there were no impacts or injuries at all.

While traveling westbound on PA state Hwy 130, a deer ran into Tim, damaging the front bumper, grill and driver side front fender and door. Tim called 911 to have the Police address the deer which was in the road and was still alive. The Penn Twp Police euthanized the deer and removed it from traffic. They also issued a police report #1900366852. The claim has been initiated with Liberty Mutual, claim # AB949-372612. The vehicle was taken to Kenny Ross Collision center in Irwin PA. We are now waiting to be assigned to a claims adjuster so that arrangements can be made for repairs.
