12:30 PM | WV | Fisher | Blacksville | At 7:30 this morning a 16" temporary above ground waterline owned and operated by Rockwater, parted at a weld during frac operations on the Fisher Pad. The waterline operator had been pumping at 70 bbls/min for a couple of minutes when he noticed a drop in pressure and increase in flow rate. He immediately shut down operations to determine where the potential issue was. The break in the line was found above the Coastal Pad, along the access road that runs between the Fisher and Coastal Pads. We are estimating that 450 bbls of a 30% concentration mixture of produced and fresh water escaped and ran down the hillside and lease road. There are no streams or waterways that connect with the contaminated area. DEP inspector Gayne Knitowski was notifed at 9:00 am this morning, and visited the location. He was supportive of the actions we had taken and asked to be kept in the loop with the NGE plan of action for remediation. DEP spill hotline was contacted at 10:30 am and assigned the following reference number 31-67939. The release was just uphill from the county road, which would be on the Coastal side. The roustabout went there immediately to evaluate the situation. The roustabout returned and gathered material to contain the unintended release. Peat Moss and oil soak rags were placed to contain the fuel. I estimate that the release was under 5 gallons. Producers had just received a delivery from Stewart Oil. The fuel delivery truck off loaded, there was a Producers employee watching and assisting the driver. There were no issues during or immediately after the delivery. Producers field supervisor called the delivery driver and asked him if he had an unintended release. The driver stated that he did not believe so but would pull over and inspect his truck. The Fuel delivery driver did call back and stated that his lower vapor line cap was off. He also stated that the vapor line will sometimes collect a little fuel when he off loads a load of fuel. The driver also states that the cap was on when he did a pre-trip prior to leaving. The poly line was spray painted with orange to identify the area for future inspections. The waste was placed in a leak proof container and returned to the Coastal pad for proper disposal. The driver also states that the fuel truck was ahead of him on the lease road. This allowed us to have a very accurate time line for the release. When the Producers guys found it there was a large quantity of FR on the containment, pumps, pump room and a small amount beside the containment. The small amount off containment has been soaked with moss and will be picked up and contained. The containment is being cut out and repaired were it is soaked. NNE's HSE rep and Water rep went to site to assess situation and put containment under the leak to prevent further spreading. Water transfer crew members were immediately called to break down the flange and identify the failure. Upon breaking the flange down, it was determined the failure was a broken bolt that holds the flange together. 02:15 PM | WV | Coastal Tank Farm | Blacksville | At 10:15 AM, April 3, 2017, NNE found a failure in the tank containment on the Coastal Pad. The failure was a six inch cut in the containment, which allowed standing rain water to exit onto the pad - all water released remained on the pad. Notifications were made, the water was tested and results were within the allowable parameters, a sand bag was placed on top of the cut to reduce any additional water from escaping, and crews were dispatched. Daybrook Development was called to remove all remaining water from containment, and RW Products was called to patch the failed portion of the containment. 04:00 PM | WV | Coastal-Campbell Access Road | Blacksville | On 12/13/17 at approximately 11:00 AM, water transfer crew members were relocating a pump from the Campbell Pad to the Dunkard Creek Withdrawal Site and upon transporting the pump, diesel fuel leaked out of the pump's fuel cell in two places along the Coastal-Campbell Access Road. Oil soak rags were immediately placed on the spill areas. The pump was taken to a level area in order to run the pump and use the extra fuel prior to transporting further on uneven terrain and potentially causing further leaking.
