Performing a pad walk around. The vacuum truck personnel reported a diesel spill on pad where personnel vehicles park.

The vacuum truck guys just reported a fuel spill in the “parking area’. This area is usually crowded with vehicles, especially at crew change. Needless to say, no one will admit to the spill. I estimate probably 3-6 gallons. The way the fuel ran, I think someone was back in parked there with either a “drop tank” or a 5 gallon type can. I think it happened after crew change, no fuel was tracked off of location.

The source of the spill is unknown at this time. The spill was cleaned up and contained. The contaminated gravel and soil has been picked up and disposed of in the environmental dumpster.

As we finished Coastal 6H stg 13, one of the hands called on the radio to tell us there was an FR spill. Went back and checked, a load of FR was delivered during the stage, the driver was told not to blow off until we were finished. (it causes the FR pumps to lose prime). Evidently, he left the pump running and left. I don’t know where he went. When the Producers guys found it there was a large quantity of FR on the containment, pumps, pump room and a small amount beside the containment. There is even a thin film on the side of the AST.

Driver – Richard Charles says he owns Charco Three the trucking company. Truck Tag AG 68768 Pennsylvania Trailer Tag PT 672E2, Copy of CDL attached, did not have any proof of insurance
Hauling for – Flowtek (FR supplier)

When asked why he was pumping after being told not to pump, he claimed no one told him. I believe this statement to be an absolute untruth. I asked him his name, I asked his employer and he told me Flowtek, I asked for his supervisors number and he gave me Rick Shaner 412-452-4032. I called Rick and found out Richard doesn’t work for Flowtek. I doubt almost everything the guy has told me.
It appears Richard Charles owns Charco Three (trucking company) and is self-employed.
Richard was told not to leave the pad until Producers was through with their investigation, he tried to get up the hill 3 times, powering out each time, before Producers employees got him stopped and backed off the hill.

The estimated volume at this time is 100 gallons until we know better. (really hard to tell how much some is on top of pumps.

We have captured as much as we can off of containment, cut the saturated felt up. The small amount off containment has been soaked with moss and will be picked up and contained. The containment is being cut out and repaired were it is soaked.

Hopefully, we will be back in operation within the next hour or two, as soon as the mat is repaired and the trucks re-spotted.

During the Patterson 334 rig DeMOB from Lemley, hauling the VFD house to the stack out yard in Mt. Morris, PA

While hauling the load down the lease road (steep blacktop portion at the bottom of Mepco), the truck began to loose traction and the load pushed the tractor around. The driver attempted to correct the situation but ended up jack-knifed. The truck came to a stop positioned over a culvert entry to a small stream. When the trailer jack-knifed, it punctured a hole in the passenger side tank and released diesel fuel. There was a nearby pallet of absorbent material. The truck driver & pilot car drivers immediately began placing pads & socks to contain the spill and to minimize the amount of fuel that made it into the culvert & stream. The truck driver was not injured.

While performing walk around found approximently 10 gal off road diesel on containment under pump #11 & #13. Spill was cleaned up, leak was found to be coming from braided fuel pump line on pump #11. Pump was shut down and line replaced. Fuel had also breached containment through a bad seam and was trapped between main containment and Felt. Containment was removed, fuel was cleaned up, rock was removed, new felt and containment was laid.

Jeff was driving by pad when he saw the leak on the 3H and ESD'd the pad.

The Mepco 3H dump valve washed out and was leaking. Jeff Wilson was driving by when he saw the leak and ESD'd the pad. Nick Swartz and Matt Jones went to Mepco and saw that the dump valve had washed out. They replaced the valve and cleaned up the site. There was a small amount of produced water that got onto the ground. The DEP inspector was notified and the area was cleaned up and new material put down. The soil was also tested.

Frac Operations on the McClellan 6H, 10H, 14H

Following up from prior report from unknown source of spill resulting in spill off containment on 2/10/21 at first was undetermined later was found to be SI SteamStream SC- 405. The leak originated on containment at a camlock on Scale line between scale ISO to blender. The camlock was not secured properly resulting in a slow leak of 600 gal of scale over the course of 12-24 hrs. The scale mixed with snow melt and rain water on containment traveled and collected at the low point of containment berm then leaking through 3 cracked seams in containment breching containment then collecting in standing water off containment. On 2/10/21 containment was cleaned and standing water directly affected off containment was picked up and desposed of in produced water tanks. A sump/trench was cut outside containment to collect and catch any residual that may accumulate. On 2/11/21 another 4-5 gal was picked up with vac truck that had accumulated over the course of 24 hrs. A 40'X70' section of containment was cut out and removed to inspect for any accumulation that could be traped under containment, no accumulation was visibly found. Containment repair replacing the removed section is underway and will be completed before moving removed frac equipment back on.

No active task at time of notification of release.

On 9/16/18, at approximately 6:25 PM, NNE was notified by a landowner that water was overflowing an air release valve (ARV) vault and running down the hill. Landowner also notified WV DEP of the incident. NNE made notifications to supervision and dispatched field personnel to visually inspect the site and ensure the leaking had stopped. Once field personnel arrived at ARV site, water was still leaking and detected a one-inch drain valve was open on the bottom of the ARV. The valve was shut and the leaking stopped. Environmental contractor was called to bring erosion controls and gather water and soil samples to begin the remediation process. NNE personnel met with WV DEP Inspector, Gayne Knitowski, to review the site and discuss next steps. Remediation efforts will be ongoing until site is vacant of all contaminated material. Spill hotline was called and NNE was provided reference number 31-80825.

Cleaning up location after rig was moved out.

A heavy storm rolled in that seized cleaning operations on the Bunner pad. Upon leaving the site, Sam noticed soap bubbles forming in the ditchline along the lease road. Nothing was present on the pad, only the bottom half of the lease road & culvert. It appeared to be Dawn soap, which is used to clean the rig prior to a move. Its assumed that a truck load leaving location the prior days splashed some residual soap onto the blacktop lease road. The heavy downpour agitated the soap and made bubbles visible. The DEP inspector was notified. The following morning the soap suds were not visible.

Rig crew instructed in the morning safety meeting to check equipment that needed to be capped and/or plugged. they men where told where their where caps, plugs and valves opened and to ensure the equipment was ready for transport prior to loading and hauling. The crew failed to ensure that the load and several others where road ready. The result was a 0.5bbl spill from the elk horn from a valve left open. The truck operated by Complete was loaded and hauling to Cummins from Matteo. The NNE company man, noticed drilling fluid along the top left side of the truck bed and wheels. The Company man inspected the truck and then headed up the lease road to find where the spill was located. The company man then took pictures, contacted NNE Drilling Engineer and went to Matteo to speak with the Patterson Rig Manager to make a plan for containment and cleaning.

A 0.5bbl spill occurred just off the Kassay Pad on the lease road heading down hill towards Cummins Pad due to the failure to properly inspect equipment prior to loading. Spill was contained and cleaned up quickly.

Was notified of soap suds in run off water believed to be coming off the pad on a neighbors property. Went to investigate.

Upon skidding the rig this morning according to Decker drillings superintendent it is believed that the rig hands disconnected the mist pump lines on the containment. The mist pump lines contained soapy water. We had a torrential downpour. All water went to the back end of our containment which is the lowest point of the burm over flowing into the gravel. The max concentration soap to water is our mist tank is 30 Bbl's of fresh water to 2 gallons off soap. Corrective action was discussed with Deckers superintended, tool pusher and rig crew that anytime lines are disconnected they will be sucked out with the super sucker also additional catch pans will be place under the tote were soap is collected on the the containment.

During an inspection of the backyard area, stressed vegetation was noticed on the backside of the Boggess pad. It appeared and was later confirmed that the source of the spill was base oil/drilling mud.

A flow line connecting the mud pits had a slow leak and had been dripping for some time. The location of the line is not readily visible without climbing between the tanks. The entire backyard area is on containment and matted. Unfortunately the leak accumulated over time and found its way under the containment due to a small hole under the matting. I contacted WVDEP, Gayne asked that we get the spill contained to make sure that it does not leave our LOD, and gave us permission for now to continue with drilling operations. Clean up will be done once the rig is moved off of location.

Pumping slug to finish tripping out of hole

While tripping out of the hole, the fluid flow path was connected to the trip tank as normal. When it was time to pump a slug, the Driller called on radio for derrick hand to line up on the flow line and off the trip tank. The derrick hand confirmed the flowline was ready. Driller began pumping and immediately noticed an increase in pump pressure and shut off pumps. The back yard came across radio and said he was seeing mud coming across the choke skid. After investigation the main flowline valve was left closed causing pressure to build up and blow out the seal between the flowline and elkhorn. There was a miss communication, and the motorman shut the valve while the derrick hand was in the derrick tripping pipe. The derrick hand assumed the valve was still open.

While conducting a field review I noticed a spill on the access road. I contacted Chris Patterson to check on any reported water truck spills. The majority of the traffic at this time is water trucks moving water from the Fisher pad, and DTE coopers run facility upgrade. Chris checked with the field coordinators, and received 2 conflicting reports claiming to have witnessed the spill. One report from ECM claimed that a Daybrook water truck leaked the fluid, and the other Report from Ransom claimed that a truck hauling a gooseneck trailer spilled the fluid. The spilled substance had a petroleum odor, but was green in appearance, could have been oil or a mixture.

Mike Shreve called out Chisler Brothers to cleanup contaminated materials , and replace with fresh gravel. Cleanup was completed within 2 hours of finding spill.

Bear Contracting, who is building the Cummins Pad, stores the diesel fuel to refill their equipment on the Kassay Pad in a vessel. (The Cummins Pad entrance begins at the northern end of the Kassay Pad). Woodford, a subcontractor of Bear, was offloading diesel fuel with a fuel truck via hose into the vessel.

Summary of events occurred Monday, April 15, 2019.
Spill Area Approximately 10' by 10', 50 gallons, diesel fuel.
Tracy Davis, Bear Cummins Pad Superintendent, was not on the physical location of the fuel tank. When he contacted me at approximately 12:00 PM he informed me that the truck driver/delivery man for Woodford, a subcontractor of Bear Contracting, had let the on site fuel tank overrun during this mornings fill up. This occurred at approximately 11:15 Am. The secondary containment was either overrun or spray from the full vessel was discharged onto the Kassay well pad outside of the secondary containment for the vessel . (Visual estimation of 50 gallons). It is not known how much freeboard was available in the secondary containment prior to incident. It is believed that no fuels left the Kassay Well Pad containment per Tracy Davis. Mr. Davis indicted the driver for Woodford was not cooperative in providing any information regarding the incident. Mr. Davis is to provide a detailed report via email. Also, NNE employee Jeff Wilson, was on location during the incident. Please see Mr. Wilson's comments regarding the incident via email attachment. Delivering base oil to the Cummins Pad

While delivering base oil to the HZ rig on the Cummins pad, the truck blew a radiator hose traveling inbound to the Matteo/Kassay/Cummins pads. As he reached the hilltop, he saw steam coming from under the hood. The driver continued up the hill to a safe spot to pull over just before the Matteo Pad next to the guard shack. The driver used absorbent pads to clean up the radiator fluid. Company man made several passes up and down the road searching for any additional spillage but was unable to locate any. Company man had driver back up truck, place new absorbent pads underneath, cleaned up all radiator fluid on ground, and the driver waited for mechanic to arrive to do repairs. Less than 1 gallon was visibly on the ground and was immediately cleaned up.

Following up on notification that a diesel spill had occurred at the Booster #2 pump station on the Mon River Waterline.

On 03 Jan 19, at approximately 1:00 PM, a diesel spill was discovered at Booster #2 pump station on the Mon River Waterline, outside secondary containment and estimated to be 5 gallons. No waters of the state or US were affected. It was determined this occurred near the end of the last transfer activity and went unreported. After discovery, a crew was dispatched to remove the contaminated stone and replace with new stone. The Spill Hotline was called and provided the following reference number: 31-84925. DEP Waste and Water Inspector Travis Hayes is also aware of the spill and the remedial actions taken.

While performing a site inspection, Josh Frazier noticed that there was a track of fluid spilled onto the Jenkins location road staging area.

After the rig move, Josh Frazier pulled onto location to perform a safety check and noticed a track of fluid where we had staged some of the Patterson equipment for the move, He notified supervision which in turn notified NNE Compnayman Josh Grim. Josh went out to check the area and get Chisler Brothers out to perform the clean up. By the time I had reached the location Chisler was already there and had absorbant on the spill and were prepping to scrape up any contamination and haul it off to be disposed of.

Unloading water recovered from drilling from water trucks into AST on Coastal pad.

Driver was unloading when he noticed a leak on the ground. After further investigation, he noticed a wing nut on the truck's pump had broken, allowing the pump to leak. When the leak was discovered, the driver put down absorbent pads and notified his supervisor. Supervisor and HSE rep went to site to conduct clean-up.

Moving around immobile equipment in tight space

The Skidsteer was non functional in the front of the drive over. The derrickman attempted to move around the skidsteer with the extend a boom forklift as he needed to get to the other side of location. The rear right tire tagged the 4" mud line going to the drive over causing the hose to separate at the crimped collar causing a 1 to 1-1/2 bbl spill. The ground below and around the spill was excavated to remove all contaminants.

Crew was on the Coastal Pad removing solids from the AST, loading into vac boxes, and making ready for transport to disposal.

After the solids removal crew loaded one of the vac boxes, it was relocated from the work area to the staging area to wait for transport to disposal. As it was being unloaded from the roll-off truck to the staging area, the hatch door wasn't fully closed and allowed some of the solids/sludge to escape and spill onto the pad, outside secondary containment. The solids removal crew cleaned up the sludge and dressed in the area with new stone.

Water line ARV inspection

While inspecting the Kassay MEPCO line on night shift, a spill was observed coming from the ARV located on the Eddie Sine property. The ARV was locked and other than the water leaking out of the vault appeared to be in good condition. Attempts were made to close the valve to shut off the flow of additional water. Anthony Carlson was notified and drove out to location from his home, he was able reach into the vault while full of produced water and shut the valve below the ARV assembly, which stopped the flow of additional water at approximately 10:30 pm Friday night.

Conducting bi-monthly AST inspection.

While conducting the bi-monthly AST inspection, water was observed to be leaking on the outside of the AST and onto the ground. All water is within the AST's secondary containment and did not leave the site. The cause of the leak is undetermined at this time, until further investigation. The AST will be drained and the liner will be inspected for a failure. Once the failure is located, a repair will be made. If no failure is found, the liner will be replaced in its entirety.

Hauling Fuel

At approximately 8:30 PM, Rock water’s fuel truck reported seeing a fuel spill on the lease road by the county road. The release was just uphill from the county road, which would be on the Coastal side. The roustabout went there immediately to evaluate the situation. The roustabout returned and gathered material to contain the unintended release. Peat Moss and oil soak rags were placed to contain the fuel. I estimate that the release was under 5 gallons. Producers had just received a delivery from Stewart Oil. The fuel delivery truck off loaded, there was a Producers employee watching and assisting the driver. There were no issues during or immediately after the delivery. Producers field supervisor called the delivery driver and asked him if he had an unintended release. The driver stated that he did not believe so but would pull over and inspect his truck. The Fuel delivery driver did call back and stated that his lower vapor line cap was off. He also stated that the vapor line will sometimes collect a little fuel when he off loads a load of fuel. The driver also states that the cap was on when he did a pre-trip prior to leaving. Steward oil also said that they will utilize a trailer that does not have a lower vapor line from now on.
The release of fuel was cleaned up by Producers employees and the roustabout. They used an excavator and shovels to scrap the area. Peat moss was also utilized to help in the collection. Fresh gravel was also replaced on the lease road. Producers supervisor and the roustabout said that all the fuel was collected, and the area looked good. I will also inspect the area in the morning. The poly line was spray painted with orange to identify the area for future inspections. The waste was placed in a leak proof container and returned to the Coastal pad for proper disposal.
I personally thanked the Rock Water driver for reporting the incident which allowed us to begin the clean up immediately. The driver also states that the fuel truck was ahead of him on the lease road. This allowed us to have a very accurate time line for the release. Producers also thanked the fuel delivery truck driver for his honesty about the vapor cap missing.

Normal solids control operation pumping synthetic based drilling mud into the centrifuge for treatment.

During normal operating conditions, NOV was processing cuttings from the synthetic based drilling mud. A 4" hose that feeds mud into the centrifuge parted. Approximately 5bbls of mud went onto the matting boards below before the operator noticed and shut down the pump. All of the fluid spilled remained on primary containment. The matting boards were picked up and cleaned underneath. The inspector was notified.
