| Summary | During a lightning shutdown, Edward Sine arrived on location without checking in with the job site supervisor and began loading an empty frac tank that had a guide wire attached to the top of the tank going up to the weight bucked attached to the derrick climb assist. He than proceeded to pull forward and noticed that there was something pulling on his load. He stepped on the brake and clutch and the pad eye located on the top of the tank at the point of attachment broke releasing the cable which snapped back and wrapped itself around the boom winch at the crown of the rig as rig personnel returned to work from being sheltered from the storm. Work was immediately stopped and the situation was investigated by Rig manager(Steve Tichey) and Co. Man(Josh Grim). |
