While traveling westbound on PA state Hwy 130, a deer ran into Tim, damaging the front bumper, grill and driver side front fender and door. Tim called 911 to have the Police address the deer which was in the road and was still alive. The Penn Twp Police euthanized the deer and removed it from traffic. They also issued a police report #1900366852. The claim has been initiated with Liberty Mutual, claim # AB949-372612. The vehicle was taken to Kenny Ross Collision center in Irwin PA. We are now waiting to be assigned to a claims adjuster so that arrangements can be made for repairs.

Tim was driving to check on wells, traveling on Rt 28N, making a left turn onto Hoover Rd. He was stopped while waiting for traffic to clear and was struck from behind by another driver. The State Police were contacted and and Trooper Smith filed a report # PA 19-250656, phone 724-543-2011. There was damage to the rear bumper of Tim's vehicle as well as the air bag system and drivers seat belt.

Fracing Stage 42 on the Lemley 1H

During 2.0 lb 100 Mesh on stage 42 on the 1H the turbine caught fire internally. A loud noise was heard from inside the data van, and smoke was seen coming from the turbine. All equipment was immediately shut down. Power Generation also called to shutdown. At this point flames were seen coming from the exhaust of the turbine. Gas supply was shut in, and the 1H and 3H wells were shut in. All nonessential personnel were sent to the muster area, and the onsite NNE consultant called 911 and NNE management. The fire was contained inside the turbine structure, and some small spot fires were visible on the containment from leaking oil. The fire suppression system inside the turbine actuated and smothered the fire internally. The small spot fires were controlled with fire extinguishers. Once the fire department arrived, the fire had already been suppressed. Once the turbine structure cooled to 100 deg F, the fire department opened the turbine doors to confirm the fire was completely out.

Mike went to future Radiotower location to review what timber had been cut.

Walked location, came back to vehicle and ate lunch. Turned on vehicle to leave location and back up into wellhead. Got out of the vehicle to see if there was any damage. The truck tailgate and bumper were damaged, but the well head did not appear to have any damage. Notified Andy Travis of incident and called Daybrook Development, the owner of the well. Day brook inspected the well and said that everything looks to be fine. There were no injuries

Truck Driver

Tractor and Trailer hauling telehandler off site did not adhere to “no left turns” sign and turned left onto Rt 7. A rock truck came around the turn on Rt. 7 from the left having to brake excessively to keep from causing an accident. No injuries or damages reported.

Around 9:20am, a lowboy with large equipment was turning left out of the Boggess well pad entrance. As the lowboy was turning out, a stone truck was traveling north on Rt 7 and when they came around the corner they had to slam on their brakes and came close to impacting the lowboy. Mike was 2 trucks behind the stone truck and had to slam on his brakes as well to avoid an accident. Mike relayed that there were no impacts or injuries at all.

Loading up Stallion living quarters.

While winching on a stallion living quarters, the dogs on the trucks winch released and allowed the trailer to drop to the ground, trailer fell approximately 1 foot. Inside of living quarters the ceiling tiles fell down and the toilet was shattered and the sink broke.

Drill Out Ops - Completed sweep at PBTD, Beginning trip out

Night shift had drilled all 59 plugs. Pulled and layed down Jt 635. Made up Jt 634 on the swivel to begin the surface to surface sweep while working the PH6 work string. At this point the saver sub and joint 634 seperated. The mud pump was SD and the pipe ram on the BOP's was shut. The WS fell and lodged into the annular bag which in return damaged the bag. Once everone was asked if they were OK the snubbers screwed a pup jt into the WS and pulled it back into the heavy slips and made up the kelly line to a Pump In Sub/TIW valve on joint 634 and continued the surface to surface sweep. Once the STS was completed we pulled Jt 634 and put it out of service.

Well Tender

Kassay 5H had a valve that cut out and was releasing gas. The men from the rig staying there heard and smelled gas and ESD'd Pad. As they ESD's the pad.......there was also an alarm for low separator pressure from the vessel which would have ESD'd the well. Steve went to location, Identified the problem and fixed the valve.

Pressure washing the driller side top sub.

While pressure washing the top sub on the drillers side of the floor, the door broke off of the hinges and fell to the ground roughly 20FT. Upon inspection the top hinge was found to be broken off for what appears to be a while due to the weathering and all that was holding the door on was the one inch piece of hinge on the bottom of the door. The impact of the water from the rig pressure washer was enough to finish breaking that hinge and the door than fell freely to the ground below. Approximate weight of the door is 80lbs. The area below the door was clear due to the buffer zone for the man lift.

While pressure washing sub with man lift, water was sprayed onto the door on the top sub and the door fell off and to the ground roughly 20ft. There was nobody near where the door fell or in the buffer zone of the man basket.

unload roller truck holding two upright clean harbors tanks.

While pulling the two Clean harbors tanks off of the roller truck to stage at the Kassay Pad, the operators(Chris Wayne, Jake Fields, and Donnie Shamblin) chained both tanks together to pull them off of the truck. The front tank began to slide to the right and fell uncontrolled to the ground off of the truck damaging the top of the tank and the valve assembly at the bottom.

Loading a frac tank onto a bobtail truck to haul from the Boggess Pad to the Cummins Pad.

During a lightning shutdown, Edward Sine arrived on location without checking in with the job site supervisor and began loading an empty frac tank that had a guide wire attached to the top of the tank going up to the weight bucked attached to the derrick climb assist. He than proceeded to pull forward and noticed that there was something pulling on his load. He stepped on the brake and clutch and the pad eye located on the top of the tank at the point of attachment broke releasing the cable which snapped back and wrapped itself around the boom winch at the crown of the rig as rig personnel returned to work from being sheltered from the storm. Work was immediately stopped and the situation was investigated by Rig manager(Steve Tichey) and Co. Man(Josh Grim).

Hauling stallion trailer to Stallion yard from Boggess Pad.

Truck driver picked up a Stallion crew accommodation that was not being used after offloading a trailer at the Mateo pad. The driver exited the pad location and drove down to route 7. The AT&T fiber line was hanging low about 14' and the driver hit the line pulling the line anchor out of the power line post. The driver then turned left onto route 7 ignoring the posted NNE signage that say's (no left turns). The Pilot car company (X5O) did not high pole the line, reason unknown.

Rigging down water transfer after drillout operations concluded.

Jeff Wilson had suspicion that Jessie Botti, an SES employee, was under the influence of something based on a conversation and observing his actions, so he called the employee's supervisor to determine. The supervisor had reasonable suspicion that he was under the influence and took him for a drug screen. The employee had a BAC of 0.23 (legal limit is 0.08) and was immediately terminated.

"This morning on Mepco I arrived on location and had a boy from Select approach me and started asking me questions about his work for the day. He didn't seem quite right so I kept an eye on him . He seemed like he was having trouble holding his train of thought. When he would bend over he would lose his balance and have to catch himself.
I contacted Cassey and told him I didn't want to accuse the boy of being under the influence of anything not knowing the boy. I asked Cassey to send someone to location that knew him to just have a conversation with him and get their opinion. Another boy showed up and I explained to him not to ask if he had been doing anything or accuse him of anything but just get him in a truck and talk to him. After 5 min the boy came to me and said that he believed he was under the influence of something and that they were taking him for drug screen. He said the boy was having some difficulty speaking and when he was asked to fill out JSA he couldn't write. The boy left with him and they were meeting Cassey and cassey was taking him for drug screen."

Well Tender

Pulled up on pad and thought a dump was stuck open. Went into unit and ball valve blew out side. Shut in Unit and the unit blew down. Within a couple of hours later another ball valve blew and a 2" Balon Dump Valve. The pad was then shut in until all units have been repaired and inspected.

As I was coming up the lease road towards the pad, I noticed a big fog above the pad and heard a very loud hissing sound as well. As I got closer I could barely see a few people off to the side of Pad and went over to see what was going on once I saw that it was safe. Brian was pretty shook up and said another dump/bypass valve had blown apart on the GPU (#4) while he was standing inside of it. He was hit by some sand, water and some loud noises but is fine as of now. With help from Dave and the ShalePro guy on location, they isolated the unit. There was also a Nalco employee taking samples right before the incident. Luckily, no one was injured.

Found a dump had blown out bottom and through top of reservoir inside GPU.

Found a dump had blown out bottom and through top of reservoir inside GPU. Gas was blowing out of unit so the ESD button was pressed to shut in pad.

Driver had just offloaded fuel on the Mepco pad and was leaving on the lease rd. The driver stated he reached down for his Pepsi and the truck dropped off the left side of the road and bottomed out in the ditch line. Hillbilly towing was called and was able to remove the fueler from the ditch.

While traveling to the field, I was in Morgantown traffic, at a standstill and was rear-ended by a pick up truck. There were no injuries or other parties involved. We pulled off of the busy road and called the police to do an accident report. The other driver was found to be at fault and cited with a warning. Pictures and a copy of the accident investigation report are attached to this report.

Jay Hewitt was a passenger at the time of the incident.

Moving center tub out away from the rig.

Charles (Night Tool Pusher) was operating the John Deere front loader to move the center tub away from the rig to pull the HCR off the mud cross. Placed forks underneath the center tub on ODS of the rig. While backing up the front steer tires transitioned from the matting boards to the cellar plates. Deflecting the cellar plates the forks tilted down and slid the center tub off. Tub slid down and pushed the corner of the rig mats into the 5H cage. The impact moved the cellar cage 2' contacting the 2" casing pressure line. Pulled the threads off the 2"nipple between the valve and elbow releasing ~600 psi of pressure. Charles immediately moved the fiberglass cellar cover and closed the valve.

While moving the center tub my forks tilted forward after driving onto the cellar plate. Causing the tub to slide off the forks and push the matting boards into the cellar cage. I heard a hissing noise and immediately moved the grating and closed the 2" valve in the cellar.

Driving over cellar with (2) 1/2" cellar plates with a full load.Tool Pusher notified Ross Drnjevich immediatly after the incedent.Do not drive over cellars with any equipment. Place equipment between cellars on solid ground. Delivering soap tote to location

As the Bill's Bit service driver(William Abraham - (412)518-8405) was pulling onto a congested location, he thought he could squeeze through a tighter spot with his truck and trailer and grazed the bumper of a parked pickup truck belonging to a floorhand(Grover Clawson - (740)434-32432).
