The driver hooked up to the ground manifold and started to pressurize the line, at which point the hose between the truck and the ground manifold failed, specifically at the cam lock connection. Approximately 1 BBL of produced water was sprayed onto the ground, outside secondary containment, but remained on pad. The driver immediately shut the truck off and removed all standing water with the truck. Calls were made to supervisors, as well as the District WV DEP Inspector. These fractures allowed freshwater to be released onto the ground. Testing was immediately halted, the line was evacuated, and repairs were made and have been scheduled. The pipe that was delivered for this installation was used previously for other transfer jobs. The fusing machine was removed from site, absorbent pads were placed on the contaminated area, and a crew removed the contaminated stone and replaced with clean stone. 12:30 PM | WV | Fisher | Blacksville | At 7:30 this morning a 16" temporary above ground waterline owned and operated by Rockwater, parted at a weld during frac operations on the Fisher Pad. The waterline operator had been pumping at 70 bbls/min for a couple of minutes when he noticed a drop in pressure and increase in flow rate. He immediately shut down operations to determine where the potential issue was. The break in the line was found above the Coastal Pad, along the access road that runs between the Fisher and Coastal Pads. We are estimating that 450 bbls of a 30% concentration mixture of produced and fresh water escaped and ran down the hillside and lease road. There are no streams or waterways that connect with the contaminated area. DEP inspector Gayne Knitowski was notifed at 9:00 am this morning, and visited the location. He was supportive of the actions we had taken and asked to be kept in the loop with the NGE plan of action for remediation. DEP spill hotline was contacted at 10:30 am and assigned the following reference number 31-67939. The pump was taken to a level area in order to run the pump and use the extra fuel prior to transporting further on uneven terrain and potentially causing further leaking. Producers had just received a delivery from Stewart Oil. The fuel delivery truck off loaded, there was a Producers employee watching and assisting the driver. There were no issues during or immediately after the delivery. Producers field supervisor called the delivery driver and asked him if he had an unintended release. The driver stated that he did not believe so but would pull over and inspect his truck. The Fuel delivery driver did call back and stated that his lower vapor line cap was off. He also stated that the vapor line will sometimes collect a little fuel when he off loads a load of fuel. The driver also states that the cap was on when he did a pre-trip prior to leaving.
