Crude Oil Soil Contamination at the Harvey 1 Well

Initial contact made by DEP Inspector, Gayne Knitowski, on March 2nd regarding oil contaminated soil in the immediate area around the wellhead. Also mentioned oil and grease on the wellhead and stuffing box. This was pre-existing condition of the well prior to acquisition by Northeast.

Jeremiah Wilt cleaned wellhead and immediate soil area around the wellhead on March 6th. Gayne was notified via email that day with accompanied pictures of site.

Gayne re-visited location on March 21st and raised concerns about remaining contaminated soil as well as berm condition of old tanks. He asked that more remediation efforts be done on contaminated soil and repair of tank berm to avoid threat of violation.

NGE Environmental onsite morning of the 22nd to obtain samples/pictures of contaminated soil and develop a plan to remediate areas of concern. Plan to pursue tilling of contaminated soil, liming, and re-seed in accordance with regulations.

Daybrook Development arrived at the Coastal AST site to offload production water into the Coastal AST. The driver hooked up to the ground manifold and started to pressurize the line, at which point the hose between the truck and the ground manifold failed, specifically at the cam lock connection. Approximately 1 BBL of produced water was sprayed onto the ground, outside secondary containment, but remained on pad. The driver immediately shut the truck off and removed all standing water with the truck.

Calls were made to supervisors, as well as the District WV DEP Inspector. NNE's first response environmental clean-up vendor was contacted and dispatched for remediation services.

DEP Inspector agreed with NNE's reactionary measures and said there was no need to call WV Spill Hotline.

At approximately 12:00 PM, while crews were transferring freshwater to Dyna Tank #1, the pressure gauge on the booster pump at the Matteo site failed. As a result, approximately 1 gallon of freshwater was released onto the ground, but within the pump's containment. The Rock Water employee manning the pump immediately shut down and notified NNE's Water Consultant. Upon investigation, it was discovered the housing for the stem of the gauge failed, not the gauge itself. Crews replaced same and resumed transferring water.

The failure was a six inch cut in the containment, which allowed standing rain water to exit onto the pad - all water released remained on the pad. Notifications were made, the water was tested and results were within the allowable parameters, a sand bag was placed on top of the cut to reduce any additional water from escaping, and crews were dispatched. Daybrook Development was called to remove all remaining water from containment, and RW Products was called to patch the failed portion of the containment.

NNE's WV DEP Inspector was called to provide a summary of the incident, and no further direction was given.

Per Site Supervisor Glenn Boyd:
One of the Flynn trucks malfunctioned overnight and they made plans to switch it out today.
A replacement truck that was apparently not road legal showed up today on a Low Boy.
The Flynn Crew transferred the fuel from the lame truck into the replacement truck.
Then they loaded the lame truck on to the low boy and removed it from location.
The Flynn Crew then spotted the replacement truck near the blender. When the pump was kicked in the swivel connection on the hose started leaking. The crew stopped transferring fuel.
We stopped operations and the entire crew started containing the fuel.
Most of the fuel was on a secondary duck pond. A small amount was on the primary containment.
All the Fuel was contained and cleaned up.
Flynn will be replacing all the oil soak pads and removing the used oil soak pads from location tonight.
It is difficult to estimate the amount of fuel lost, but it may be as much as 40 according to Flynn.

I just received word from the field that one of the water truck drivers from Ransom Drilling was killed as a result of his truck wrecking. I?m told as he was traveling on Rt. 50 to/from one the disposal facilities in Ohio, the truck blew a tire and he lost control. DOT is on scene and will conduct an investigation. There is a story already on WBOY?s website with two pictures, as well as WDTV?s website.

When transporting SBM tank from Fisher to Coastal the truck started down hill and about 1 bbl of SBM splashed out of vent cap on top of tank. It then began to drip down on the lease road. It remained contained to the lease road.

While performing our bore at the Jakes Run Stream Crossing we have seeped into the stream bed with our drilling mud. The mud finds a way to the surface from the bore. Bentonite is the only foreign substance in the mud. The stream is milky in color. I notified WVDEP inspector. Inspector Arrived on site at 2:10pm
Seep location was Identified. Our plan we presented to the inspector upon arrival as follows:
Build a temporary coffer dam above seep area with flume pipe. This construction practice has been approved per construction detail located on Sheet D10 of the approved NPDES plans. This is also acceptable with the ACOE due to ?discharges into waters are under 500? or 1/10th acre for each crossing? per email approval from the corps for the project.
The inspector is OK with this plan,as long as it was in the scope of our permit, coffer dam and flume will be built and operations will resume.
Hollie notified the spill hot line.

While delivering a load of fuel to location, diesel was spotted underneath the fuel truck every time he came to a stop. We followed the truck and immediately got him up on containment and placed a drip pan under the leak. Once the leak was contained we began immediate transfer of fuel from the truck to the diesel tank so we could empty out the fuel load. The drivers name was Rick Bowser (330-847-0087) driving truck #25, He is employed by Mike Ainsley (330-727-1414) of Ainsley oil and has 3 Yrs with the company, 15 years driving experience.

On 11/21/17 at approximately 4:15 PM and on 11/22/17 at approximately 2:00 PM, as crews were pressure testing the temporary, aboveground waterline between Coastal and Fisher, stress fractures were discovered during the line walks. These fractures allowed freshwater to be released onto the ground. Testing was immediately halted, the line was evacuated, and repairs were made and have been scheduled. The pipe that was delivered for this installation was used previously for other transfer jobs. QAQC was performed on the pipe prior to installation, but due to the nature of these impairments, they could not have been detected until pressure was applied to the line.

On November 30, 2017 at approximately 3:15 PM, NNE employee arrived onsite at the auxiliary pad above the Fisher Pad and found hydraulic fluid released on the ground where a fusing machine had been sitting. The fusing machine was removed from site, absorbent pads were placed on the contaminated area, and a crew removed the contaminated stone and replaced with clean stone. WV DEP Inspector, Gayne Knitowski, was notified and provided no other action to take.

At 7:30 this morning a 16" temporary above ground waterline owned and operated by Rockwater, parted at a weld during frac operations on the Fisher Pad. The waterline operator had been pumping at 70 bbls/min for a couple of minutes when he noticed a drop in pressure and increase in flow rate. He immediately shut down operations to determine where the potential issue was. The break in the line was found above the Coastal Pad, along the access road that runs between the Fisher and Coastal Pads. We are estimating that 450 bbls of a 30% concentration mixture of produced and fresh water escaped and ran down the hillside and lease road. There are no streams or waterways that connect with the contaminated area. DEP inspector Gayne Knitowski was notifed at 9:00 am this morning, and visited the location. He was supportive of the actions we had taken and asked to be kept in the loop with the NGE plan of action for remediation. DEP spill hotline was contacted at 10:30 am and assigned the following reference number 31-67939. NGE has been brought in to address environmental remediation, and we are scheduling a meeting with Rockwater to address this incident.

On 12/13/17 at approximately 11:00 AM, water transfer crew members were relocating a pump from the Campbell Pad to the Dunkard Creek Withdrawal Site and upon transporting the pump, diesel fuel leaked out of the pump's fuel cell in two places along the Coastal-Campbell Access Road. Oil soak rags were immediately placed on the spill areas. The pump was taken to a level area in order to run the pump and use the extra fuel prior to transporting further on uneven terrain and potentially causing further leaking. NNE's HSE Rep and Water Rep talked with crew members about fueling with consideration of transporting pumps.

On 12/13/17 at approximately 1:30 PM, NNE's drilling consultant observed water leaking at a flange on the discharge pipe at the AST. An estimated 2 BBL of blended water was released onto the ground, outside of containment. The water never left the well pad and did not impact any streams. NNE's HSE rep and Water rep went to site to assess situation and put containment under the leak to prevent further spreading. Water transfer crew members were immediately called to break down the flange and identify the failure. Upon breaking the flange down, it was determined the failure was a broken bolt that holds the flange together. Crew members replaced the damaged bolt with a new one, bolted the flange back up, which resolved the issue.

While conducting a field review I noticed a spill on the access road. I contacted Chris Patterson to check on any reported water truck spills. The majority of the traffic at this time is water trucks moving water from the Fisher pad, and DTE coopers run facility upgrade. Chris checked with the field coordinators, and received 2 conflicting reports claiming to have witnessed the spill. One report from ECM claimed that a Daybrook water truck leaked the fluid, and the other Report from Ransom claimed that a truck hauling a gooseneck trailer spilled the fluid. The spilled substance had a petroleum odor, but was green in appearance, could have been oil or a mixture.

Mike Shreve called out Chisler Brothers to cleanup contaminated materials , and replace with fresh gravel. Cleanup was completed within 2 hours of finding spill.

Following up on notification that a diesel spill had occurred at the Booster #2 pump station on the Mon River Waterline.

On 03 Jan 19, at approximately 1:00 PM, a diesel spill was discovered at Booster #2 pump station on the Mon River Waterline, outside secondary containment and estimated to be 5 gallons. No waters of the state or US were affected. It was determined this occurred near the end of the last transfer activity and went unreported. After discovery, a crew was dispatched to remove the contaminated stone and replace with new stone. The Spill Hotline was called and provided the following reference number: 31-84925. DEP Waste and Water Inspector Travis Hayes is also aware of the spill and the remedial actions taken.

Unloading water recovered from drilling from water trucks into AST on Coastal pad.

Driver was unloading when he noticed a leak on the ground. After further investigation, he noticed a wing nut on the truck's pump had broken, allowing the pump to leak. When the leak was discovered, the driver put down absorbent pads and notified his supervisor. Supervisor and HSE rep went to site to conduct clean-up.

Pumping slug to finish tripping out of hole

While tripping out of the hole, the fluid flow path was connected to the trip tank as normal. When it was time to pump a slug, the Driller called on radio for derrick hand to line up on the flow line and off the trip tank. The derrick hand confirmed the flowline was ready. Driller began pumping and immediately noticed an increase in pump pressure and shut off pumps. The back yard came across radio and said he was seeing mud coming across the choke skid. After investigation the main flowline valve was left closed causing pressure to build up and blow out the seal between the flowline and elkhorn. There was a miss communication, and the motorman shut the valve while the derrick hand was in the derrick tripping pipe. The derrick hand assumed the valve was still open.

Re-fueling a portable heater unit.

"While re-fueling a third piece of equipment (a portable heater unit) the transfer / pump hose split and began to spray / release diesel fuel. The employee immediately shut down the pump and began to place spill pads to absorb the fuel. The fuel was on the bed of the truck as well as on the ground. Before the truck was moved it was cleaned to verify no additional areas would be affected. Approximately 2 - 4 gallons of diesel fuel was released (see attached photos). The spill area was cleaned up by placing the soiled spill pads and any dirt / rocks, that were affected, into collection buckets for disposal. The hose transfer line on the fuel cell was immediately replaced."

Reclamation of Paw Paw Creek road bore

While reclaiming the Paw Paw Creek road bore, the operator struck an unmarked 3" water line . Fairview water department was notified and shut the water off after approximately 1 hour. There was enough water that the super silt fence and silt sock were compromised and allowed fresh water to flow into Paw Paw Creek.

While boring under 218/Paw Paw Creek Rd our drilling assembly became plugged with cuttings. In an effort to regain circulation a smaller assembly was inserted to remove the plugged section. During this operation the plug released and with it so did the drilling mud and spring water that had built up behind the plug. This volume of fluid overcame the drill pit, reserve pit and vacuum truck , as well as 2 layers of silt protection and an estimated 35 bbls of fluid containing betonite clay made its way into Paw Paw Creek.

Summary from East Coast Underground driller Clint George. "We had lost flow to the drill. We put on 12” reamer and started bringing it back to the drill in attempt to get the flow back to the drill. We were approximately 155 feet away from the drill with the reamer. While drilling out we hit a spring in the hillside that
is producing approximately 5 gallons a minute. Which was adding to the fluid that we had pumped into the bore hole. When
the flow started to come back to the drill. We were not able to keep up with the flow rate that was coming out of the bore hole.
We had a Vac truck sucking water up as well as the reclaimer pump picking up water as well. The 2 of them could not keep up
with it as fast as it was coming out. The fluid over ran both pits and ran out of the last pit. To the E and S control ran over them
and ran into the stream. Releasing approximately 700 to 1000 gallons of fluid into the stream."
