12:30 PM | WV | Fisher | Blacksville | At 7:30 this morning a 16" temporary above ground waterline owned and operated by Rockwater, parted at a weld during frac operations on the Fisher Pad. The waterline operator had been pumping at 70 bbls/min for a couple of minutes when he noticed a drop in pressure and increase in flow rate. He immediately shut down operations to determine where the potential issue was. The break in the line was found above the Coastal Pad, along the access road that runs between the Fisher and Coastal Pads. We are estimating that 450 bbls of a 30% concentration mixture of produced and fresh water escaped and ran down the hillside and lease road. There are no streams or waterways that connect with the contaminated area. DEP inspector Gayne Knitowski was notifed at 9:00 am this morning, and visited the location. He was supportive of the actions we had taken and asked to be kept in the loop with the NGE plan of action for remediation. DEP spill hotline was contacted at 10:30 am and assigned the following reference number 31-67939. NGE has been brought in to address environmental remediation, and we are scheduling a meeting with Rockwater to address this incident. | 100 gallons or greater | Andy Travis

(really hard to tell how much some is on top of pumps. We have captured as much as we can off of containment, cut the saturated felt up. The small amount off containment has been soaked with moss and will be picked up and contained. The containment is being cut out and repaired were it is soaked. Hopefully, we will be back in operation within the next hour or two, as soon as the mat is repaired and the trucks re-spotted. | 300 Gallons | 298 Gallons | Spill was contained | Contacted Criss VanGuilder | Gayne Knitowski | Not Req | 2 hrs | Spill Cleaned (6 hours) | None | Pending | Shane Stephenson | Email hidden; Javascript is required. | (740) 586-8729 | (740) 454-6253 | Max Oliver | Email hidden; Javascript is required. | (817) 526-4147 | (817) 526-4147 | Criss VanGuilder | Email hidden; Javascript is required. | (405) 757-8725 | (304) 212-0423 | Hollie Medley | Email hidden; Javascript is required. | (304) 543-8932 | (304) 212-0422 | Andy Travis | Email hidden; Javascript is required. | (304) 951-0909 | (304) 212-0415 | Andy Travis

In an effort to regain circulation a smaller assembly was inserted to remove the plugged section. During this operation the plug released and with it so did the drilling mud and spring water that had built up behind the plug. This volume of fluid overcame the drill pit, reserve pit and vacuum truck , as well as 2 layers of silt protection and an estimated 35 bbls of fluid containing betonite clay made its way into Paw Paw Creek. | Summary from East Coast Underground driller Clint George. "We had lost flow to the drill. We put on 12” reamer and started bringing it back to the drill in attempt to get the flow back to the drill. The vacuum truck personnel reported a diesel spill on pad where personnel vehicles park. | The vacuum truck guys just reported a fuel spill in the “parking area’. This area is usually crowded with vehicles, especially at crew change. Needless to say, no one will admit to the spill. I estimate probably 3-6 gallons. The way the fuel ran, I think someone was back in parked there with either a “drop tank” or a 5 gallon type can. I think it happened after crew change, no fuel was tracked off of location. The source of the spill is unknown at this time. The spill was cleaned up and contained. The contaminated gravel and soil has been picked up and disposed of in the environmental dumpster. | 4 gal | No | Yes | Yes | Yes | No | Immediately | Yes | No | The source of the spill is unknown at this time. The release was just uphill from the county road, which would be on the Coastal side. The roustabout went there immediately to evaluate the situation. The roustabout returned and gathered material to contain the unintended release. Peat Moss and oil soak rags were placed to contain the fuel. I estimate that the release was under 5 gallons. Producers had just received a delivery from Stewart Oil. The fuel delivery truck off loaded, there was a Producers employee watching and assisting the driver. There were no issues during or immediately after the delivery. Producers field supervisor called the delivery driver and asked him if he had an unintended release. The driver stated that he did not believe so but would pull over and inspect his truck. The Fuel delivery driver did call back and stated that his lower vapor line cap was off. He also stated that the vapor line will sometimes collect a little fuel when he off loads a load of fuel. The driver also states that the cap was on when he did a pre-trip prior to leaving. Steward oil also said that they will utilize a trailer that does not have a lower vapor line from now on. | 5 gal | No | Yes | Yes | Yes | No | Immediately | Yes | No | The release of fuel was cleaned up by Producers employees and the roustabout. They used an excavator and shovels to scrap the area. Peat moss was also utilized to help in the collection. Fresh gravel was also replaced on the lease road. Producers supervisor and the roustabout said that all the fuel was collected, and the area looked good. I will also inspect the area in the morning. The poly line was spray painted with orange to identify the area for future inspections. The waste was placed in a leak proof container and returned to the Coastal pad for proper disposal. I personally thanked the Rock Water driver for reporting the incident which allowed us to begin the clean up immediately. The driver also states that the fuel truck was ahead of him on the lease road. This allowed us to have a very accurate time line for the release. Producers also thanked the fuel delivery truck driver for his honesty about the vapor cap missing. | Eric Stuart | Email hidden; Javascript is required. | (304) 482-1179 | (304) 482-1179 | Mike Inman | Email hidden; Javascript is required. | (989) 858-6112 | (989) 858-6112 | Andy Travis | Email hidden; Javascript is required. | (304) 951-0909 | (304) 951-0909 | Hollie Medley | Email hidden; Javascript is required. | (304) 543-8932 | (304) 212-0422 | Andy Travis | Email hidden; Javascript is required. | (304) 951-0909 | (304) 212-0415 | Criss VanGilder
