Tong Cable Repair/Rig Crown

Employee Todd Lee went to the crown of the rig to check the tong cable in the shive, The driller Eric Stanley left the rig running in case he needed to pick up the tong with the winch line to take the weight off the cable to put it back in the shive. After determining the winch was not needed for this task he started up with the winch to hook on the casing being cut on the table, at this time Todd Lee put his hand on the cable and got his right middle finger pulled into the shive crushing the tip of the finger. He was taken to the hospital and received 4 stitches, took off the nail and broke the bone in the tip of the finger. A saftey stand down was preformed and a follow up will be conducted. Updates will follow as we get them.

Mud Pump Repairs/

During the process of repairing Mud Pump #1, the Floorhand Cory Michael Chioccawas was manually turning the drum wheel to position the pump in the correct spot for repairs and rolled his right hand middle finger between the wheel and the belt of the pump. He was not wearing his impact resistant gloves. Joey Richardson(drilling Superintendent) was already on site due to the time it had taken for repairs and they began the Axiom/HSE process. As soon as HSE arrived on location, they transported him to the hospital ER for treatment. He received stitches and a split as he broke his finger. Justin Bussey(Operations Manager) and Clinton Hulslander(HSE Supervisior) were on site. The process for doing this work has been changed to keep hands and tools out of the way.

Containment Repairs

Employee Zack Anderson was operating a extrusion welder when he lost his grip on it and instinctively reached out to catch it with his other hand, by doing so he grabbed the heating element causing burns to the tips of his left thumb, index and middle fingers.

He was wearing gloves at the time. He applied ice on location but declined to go to the hospital.
Small blisters appeared on his finger tips.

He has 1 year experience and is the supervisor of the crew.

Traffic control for Fisher frac

At 4:25 am June 26, an employee with ECM twisted his ankle when stepping onto the guard shack step from the ground. The employee's boots were wet at the time and caused him to slip on the wooden floor. This was a pre-existing condition that occured off location on June 22. He had previously gone to the doctor and was instructed to wear an ankle brace, which he was not wearing at the time of re-injury. The ECM employee was relieved on pad and went to a walk in clinic for first aid.

Breaking down setting tool

At approximately 4:30 AM, on April 29th, 2017, NES was operating wireline on the Northeast Natural Energy Kassay location. NES employee was breaking down setting tool and while trying to expose piston, encountered trapped pressure, which caused the tool to not stroke. NES employee tried to make tool stroke by slamming the tool on surface containment, and while in the process, got finger pinched in the setting tool. Employee later sought medical attention at local ER.

Climbing a ladder out of the cellar.

Employee Adam Coffman was in the cellar making repairs to the diverter, once finished with his repair Adam was exiting the cellar up a ladder when he slipped on one of the rungs of the ladder and reached out to grab a rung above him. He stated at this time that it strained his arm when catching himself. The night rig manager was close to the employee when this happened. Adam said he had strained this arm 9 months ago. Adam has been a employee for 5 days.

Warren was climbing down out of his truck bed, after strapping his load down.

In the process he lost his balance and fell to the ground striking his left elbow and injuring his left shoulder. He was taken to St Josephs ER for evaluation, where x-rays determined that he had a left shoulder contusion. He was cleared to return to work 3/24 with no restrictions.

Attaching camlock hose fitting together to lengthen suction hose.

While putting together two hoses a burr went through Cornelius's glove and cut his left ring finger. General first aid requiring antiseptic cream and a band aid was administered. No further action was required.

The marooka truck was stuck and the excavator operator started to pull the truck with the tailgate and pulled the tailgate off the truck then it struck John Lamb in the leg. He was taken to the hospital and ex rays were done. He has no broken bones, just bruised.

Getting fittings to the rig floor to hook up the casing fill up line.

While carrying 2 2" fittings to hook up the casing fill up line, Kyle was walking across the floor and tripped over the hydraulic feed lines to the power tongs. When he fell he landed on his Left shoulder and separated/dislocated it.

While walking across the floor carrying fittings Kyle tripped over the hydraulic hose and fell onto his left shoulder, he instantly felt pain and had to be assisted back to his feet. He was unable to move his shoulder and required assistance down the stairs to the rig managers house where he explained his symptoms.

James was checking in personnel and administering temp check and COVID questionnaire for Lemley top hole from a guard shack positioned off location on the access road.

While leaving the shack to check in a vehicle, a weld on the steps broke causing James to fall on his right hip. He notified his supervisor and refused medical attention. He was able to finish his shift and seems to be okay. The shack is not set on level ground and had been cribbed with boards to support and level the shack.

Cleaning up location, loading truck with scrap metal

While loading a truck with a piece of flowline, Jim Farris was rolling the flowline off of the forks of the telehandler, to make sure he didn’t damage the truck. The forklift operator, Chris Robinson (Highlands Drilling) lowered the flowline without notice pinching Jim's right hand between the flange on the flowline and the trailer.

Making rounds on compressor. Taking readings and shooting valve and discharge temps.

Mark called me (John Landis) on Friday morning and stated that his left hip was hurting and he was limping around on it. I asked Mark what had happened and he told me that he was climbing up onto the compressor to close the Louvers. He stated that he felt a pinch in his left hip and thigh but continued working. When he woke up on Friday morning he had pain in his left hip and thigh. I told Mark to go home to rest up his leg, he worked until 3:00 PM and went home. He tried working the rest of the day, but the pain persisted on Friday. Mark also told me that earlier in the week (Tuesday) he hit his left hip on a valve at the Boggess Pad. We are unsure if this has anything to do with the injury, but wanted to make note of it. Mark is currently off resting his hip/thigh.

Climbing up compressor to close off louvers to adjust discharge temps. Felt a pinch in left hip and thigh but continued working. I woke up Friday morning with pain in left thigh and hip. I worked Friday until 3:00. The pain persisted so I went home.

Mark called and said that on Tuesday he had hit is left hip on a valve at Boggess. Then on Thursday while climbing up onto Coastal compressor to open the louvers he felt a pull in his left hip. We are not sure if both instances correlate. On Thursday Mark's hip was sore and then on Friday, the hip was painful and Mark was limping around on it. He called me (John Landis) on Friday to tell me about everything that had happened. I told Mark to go home and rest up his hip.

Well Tender

Jeff Krakau had called me at 7:50 stating that something may have happened to Dave Beckner (he had stopped talking on phone after hearing him say "oh"). Jeff tried calling back but no answer. Jeff called me and asked if I had talked to him. I called and texted Dave, but no answer. I decided to drive out to the dehy location to check on Dave to see if he was ok. When I arrived on location Dave was reclined in his seat in his truck and holding his left hand. His arms were folded across his belly/chest. I then tried to get Dave to respond to me and after a couple of minutes he was responsive. Dave could not talk and did not know who I was. I could see a large bump on his forehead, a scratch under his left eye. I then called 911. 911 came and took Dave to Ruby Hospital. While at Ruby they did a CT scan and saw no bleeding on the brain. As a preliminary finding they were saying they thought it to be a concussion, but more testing would take place. They were going to do an MRI and an EEG on Dave. Dave does have loss of memory and cannot speak at this time.

"Employee was manual felling an area for the future Paw Paw waterline project with another employee approx. 500 ft apart. During one of his completed tree fells approx. 14’ an unseen loose vine that was covered in leafs got intertwined in the neighboring tree approx. 4’ splitting it in half and causing it to spring toward the employee unexpectedly as the employee was traveling away from base of tree via is exit route. The impact striking him in the left side of hard hat."

"The injured employee contacted his fellow employee for help via cell phone who was located at the current site area. Contact was made upon the initial blow from the limb to the head area. That employee in turn went to immediately assist injured and called the on -site foreman gave location details and need for means of egress. Employee was then transported by utv to a waiting pickup and then transported to the nearest hospital for medical treatment/evaluation."

Kevin was clearing in the same vicinity 500'-700' away with no line of sight. He received a phone call from Wes letting him know that he had been struck. When he got to Wes, he found him sitting upright, conscious and coherent. They contacted Josh Boulet for assistance, Josh contacted Jeremy Lane. Jememy and MIke Shreve came in and removed Wes with a UTV. Kevin stated that he asked what happened and Wes was unsure.

While stepping up into the track hoe to mix off cuttings in the backyard, Michael felt a pop in his left knee and could not put his weight on that leg. Notifications were made and he was taken to the ER in Morgantown for X-rays.

While walking around on location looking for some panic lines for the choke, the injured Patterson employee stepped in a low spot and reached out to to prevent himself from falling. He reached out to grab the top of the junk tub and cut his right index finger. See photos attached.

Changing filters on outlet filter of compressor.

While changing filters Roberts hand got stuck between cap and lock down bar. He had bruising and skin abrasion. First aid was administered. He was taken to DR and it was reported as just bruising and sore. No work was missed.

While working to assist Nick form NNE and learning to change filters on the Kassay Pad, I was injured in the process of threading the end cap back on. I was pushing from the bottom with my right hand and as I pushed off one of the ears that hold the ratcheting mechanism into place my hand slipped off shooting past the hook on the ear which caught the meat of my palm pushing my hand into get crushed. Nick and I backed off the cap where Nick proceeded to check on me and provide me with a cold pack to try to mitigate the swelling and figure out what to do after.
